How do I schedule an appointment?

Where are you located? 

I am located at 8080 Beckett Center Drive, Suite 313, West Chester, Ohio, 45069.  
I am about 11.5 miles north of the Tri-County Mall.  

What populations do you work with? 

I work with children ages 4 years and up, adolescents, and adults. 

What hours are you available? 

I offer flexible hours with week day availability.

What do I need to bring? 

Print and fill out the Adult Information Form or the Child/Adolescent Form which is relative to whom is seeking services.  Print out the Explanation of Services and Informed Consent.  We will review this at the initial appointment.  If you are seeking services for a minor child please bring any documents that may be necessary to establish who has legal ability to consent to treatment for the child such as divorce decree, shared parenting agreement, or any other document.  Bring any questions you may have, I am happy to answer them.

What is the cost for services? 

Cost for an individual session is $90.00 for 50 minute session.  Family sessions are $100.00 for 50 minute session.  Sessions extended beyond are billed at $25.00 per every 15 minutes whether in whole or in part.  If the extended time is under 15 minutes, the full amount of $25.00 will be charged.

I am bringing my child/adolescent for counseling.  Should I bring him/her on the first visit? 

The first session when a child/adolescent is the client is spent with the parent/guardian.  This is a time to gather history and get the parent/guardian perspective.

Will I be informed about what is happening with my child/adolescent’s counseling? 

I typically like to spend a few minutes before or after the session with the parent/guardian.  The parent/guardian is the best support person I can have.  I welcome the involvement of a parent/guardian.  The presence of the parent/guardian in session is determined by the child’s needs, parent/guardian needs/wishes, and the therapeutic value of the parent/guardian presence which I am happy to discuss during intake.
